INSURANCE
Please bring your current insurance card to every visit. It is important that you provide correct information at check-in so we file correctly for all services (including lab work and tests ordered at other facilities).
Please be prepared to pay your copay at each visit. We will file your insurance as a courtesy to you. Any remaining balances for deductibles and coinsurance will be billed to you.
Please remember that each patient's insurance policy is different and we do not know what each one requires or covers. If you have a question about your insurance policy or coverage, please call the telephone number on the back of your insurance card.

We participate in many PPO networks. Please call your insurance company to find out whether we participate in your plan.
Workers Compensation? Auto Accident?
Were you injured on the job? Your regular health insurance usually does not pay for on-the-job injuries. You must follow special workers compensation insurance requirements. Report the injury to your supervisor within 24 hours and ask at work which physicians are on your panel for workers comp. We do not participate in any worker's compensation networks.
Please let us know for each visit if you want us to bill a different type of insurance other than your usual health insurance.
