Picture IDs Do We Have the Correct Information?
Why do we ask for a copy of your driver's license or picture ID? Because we want to remember who you are! We often have several patients with the same name. Sometimes (we hate to admit it) we can't remember what you look like. We feel it helps us provide better care and better privacy protection for you.
We want to be able to contact you if we need to, and we want to file your insurance correctly the first time. But for everything to work smoothly, we need to make sure we have the correct address, telephone numbers and insurance information at each visit. Please help us by bringing your correct insurance card to each visit and be willing to give it to us each time we ask.
